<br />Attachment A <br /> <br />Part Five - Grant Closeout <br /> <br />Notification of <br />Grant <br />Completion <br /> <br />Closeout <br />Review and <br />Notification <br /> <br />Financial <br />Reconciliation <br />of Grant Funds <br /> <br />Audit <br />Requirements <br /> <br />The grant recipient must submit a written notification to the state within 30 <br />days of the completion of all approved work and disbursement of all funds. <br /> <br />The state will review the grant recipient's fmal performance report for <br />compliance with all grant conditions. After compliance has been verified, the <br />state will notify the grant recipient of the grant closure and record retention <br />requirements. <br /> <br />If the final performance report indicates that the grant recipient: <br /> <br />· is owed additional funds, the state will send the final payment <br />automatically to the grant recipient. <br />· did not use all funds received, the state will issue anlnvoice to recover <br />unused funds. <br /> <br />Grant recipients, and any other agencies to which they subgrant funds, must <br />comply with the audit requirements contained in OMB Circular A-133 and <br />Title 28, CFR, Part 66.26. <br /> <br />FY04 Urban Areas Security Initiative <br /> <br />Page 38 <br />
