IV. Description of Work - in space below, describe the program to be funded during the 12- <br />month contract period including, services to be provided, program goals, client <br />characteristics, & how grant funds will be utilized. <br />Services to be Provide: Stop Short of Addiction -Spanish program (SSOA-Sp) includes a <br />clinical intake assessment, (3) pre -therapy sessions that are pychoeducational and process <br />oriented, up to 12 sessions of family therapy as resources allow, and an exit meeting and plan to <br />assist youth in families in maintaining a drug and alcohol free life. <br />Program Goals: The goal of SSOA-Sp is to reducerisky behaviors such as substance abuse, <br />while linking Latino youth and families to culturally appropriate healthcare services. High -risk <br />adolescents between 12-17 years and their families to accomplish the following objectives: <br />1)provide access to free substance abuse program that is culturally, language and age <br />appropriate; 2) work with underserved, Latino youth to reduce the spread of substance abuse, and <br />3) provide information critical to accessing substance abuse and behavioral health services. <br />Client Characteristics: Client's served are Latino adolescents between 12-17 years and their <br />families. Our client's have limited English speaking abilities, are of low-income status, limited <br />educations and lack health insurance. Our adolescents are at high -risk of pregnancy, drug/alcohol <br />use, dropping out of school and exposure or involvement in gangs. <br />Grant funds will be utilized: To provide 10 Santa Ana youth and 10 Santa Ana adults with <br />services through Stop Short of Addiction, Spanish program, which includes 1) clinical intake <br />assessment, 2) 3-pre-therapy sessions that are pychoeducational and process oriented, 3) up to 12 <br />sessions of family therapy as resources allow, and 4) an exit meeting and plan. <br />Grant funds will defray costs necessary to operate program <br />Schedule of Performance <br />Estimate the number of unduplicated Santa Ana residents <br />to be served during the 12-month <br />contract period per quarter: <br />Quarter 1: July 1 — September 30 <br />5 <br />Persons <br />Quarter 2: October 1 — December 31 <br />5 <br />Persons <br />Quarter 3: January 1 — March 31 <br />5 <br />Persons <br />Quarter 4: April 1 — June 30 <br />5 <br />Persons <br />Total <br />20 <br />Persons <br />Invoicing Schedule <br />Estimate the amount of grant funds to be requested during the 12-month contract period on a <br />quarterly basis: <br />Quarter 1: <br />July 1 — September 30 <br />$ <br />1250 <br />Quarter 2: <br />October 1 — December 31 <br />$ <br />1250 <br />Quarter 3: <br />January 1 — March 31 <br />$ <br />1250 <br />Quarter 4: <br />April 1 — June 30 <br />$ <br />1250 <br />Total <br />S <br />5000 <br />EXHIBIT A <br />
