Description of Work - in space below, describe the program to be funded during the 12 - <br />month contract period including, services to be provided, program goals, client <br />characteristics, & how grant funds will be utilized. <br />Congregate Meal Program <br />Purpose 6 Need - The Congregate Meal program addresses the need for older adults to have a nutritional meal. <br />However, the program Is always more than just a meal. Many Isolated older adults have the opportunity, through this <br />program, to socialize, access other services, and enjoy the company of their peers, in a safe and friendly environment. <br />For some, this is the only social¢atlon they eltperience during the day. A donation is suggested, but no ane is ever turned <br />away due to an inability to donate. Goal - Feedback Foundation proposes to serve 890 unduplicated Santa Ana seniors <br />during 2005116. Feedback Foundation anticipates serving 37,0110 meals at the Santa Ana congregate sitea which include; <br />Santa Ana Senior Center, Southwest Senior Center, and the Vietnamese Community Center. <br />Homebound Meal Program <br />Purpose & Need -The donation based home delivered meals program assists frail, at -risk older adults to remain in their <br />home following the on -set of a chronic or acute physical and/or mentally debilitating illness. Three meals are prepared <br />and dellvered Monday through Friday. Uhler supervision and guidance of our Registered Dietician, all nutrition needs are <br />addressed. The daily connection with the delivery driven and case managers allow other needs to be addressed as well, <br />There is a watchful eye on all recipients and many receive additional support from community agencies and service clubs. <br />This too, decreases feelings of isolation and loneliness.
