Number of unduplicated Santa Ana residents to be served with grant funds during the 12- <br />month contract period = 26 PERSONS <br />III. Below is the City performance outcome that our organization will be tracking. <br />COMMUNITY PRIORITY NEEDS: Public Services <br />Activity/Program <br />Outcome Tracking <br />Goal <br />Owner -Occupied Housing <br />Number of owners assisted to <br />Physical safety and comfort will be <br />Rehabilitation <br />rehabilitate their primary residence <br />improved for Low and Moderate <br />for the following income categories: <br />income households by improving <br />-Extremely Low Income <br />homes to a level that meets or exceeds <br />-Very Low Income <br />HUD housing quality standards. <br />-Low Income <br />-Moderate Income <br />Special Needs Categories <br />-Large Families <br />IV. Description of Work - in space below, describe the program to be funded during the <br />12-month contract period including, services to be provided, program goals, client <br />characteristics, & how grant funds will be utilized. <br />The descriptive goals remain the same as identified in original grant proposal: <br />1) Identify low-income and/or disabled homeowners in need of assistance to make <br />their homes safe, accessible, secure, warm and dry. <br />2) Provide the resources to rehabilitate an increased number of qualified owner - <br />occupied homes in the City of Santa Ana. <br />3) Continue development of our capability to accomplish year-round home <br />modifications for homeowners with disabilities. <br />E x A <br />
