IV. Description of Work - in space below, describe the program to be funded during the 12 - <br />month contract period including, services to be provided, program goals, client <br />characteristics, & how grant funds will be utilized. <br />The Linkages Program provides case management services and comprehensive information and <br />referrals to frail elderly and disabled persons aged 18 and above to aid in obtaining appropriate <br />community resources that prevent or delay institutionalization. To meet the needs of vulnerable adults <br />living in Santa Ana, Linkages staff will: <br />• Provide phone intake services in person during business hours with bilingual service <br />available. <br />• Responds promptly to requests for case management Wor service referral requests. <br />• Provides timely assessments and create an appropriate care -plan using an informal <br />collaborative or contractual approach. <br />• Refer caregivers to counseling, family consulting services and respite assistance. <br />In the Fiscal Year 2005-2006, with $6,000 from Santa Ana CDBG, the Linkages program will provide <br />comprehensive information, referrals and case management services to 33 unduplicated Santa Ana <br />residents. The grant funds will be used to supplement the salary of the bilingual English/Spanish case <br />manager who services Santa Ana residents.
