IV. Description of Work - in space below, describe the program to be funded during the 12- <br />month contract period including, services to be provided, program goals, client <br />characteristics, & how grant funds will be utilized. <br />Goal: To help low-income people access quality health services and navigate the delivery systems so that they can <br />obtain critical medical services for which they are entitled to as well as empowering consumers to be active <br />participants in their own healthcare and in planning programs designed to serve them. Although the HCAC provides <br />outreach services in addition to direct client services, LASOC is requesting funding to assist with direct services. <br />The following objectives relate to the direct service component of HCAC. <br />Objective(s): <br />1. Help 55 low-income residents of Santa Ana resolve problems related to accessing critical medical services, <br />which if not obtained, could increase their risk of serious illness or even death. Act as legal advocates on behalf of <br />clients to ensure that residents receive much -needed medical care regardless of income. <br />Task/Action- <br />(A) Screen applicant for eligibility through the HCAC Hotline Monday through Friday and provide advice and <br />counsel to eligible clients. <br />(B) Represent clients who need a legal advocate to intervene. <br />2. Help low-income Santa Ana Residents become more informed and educated about their rights under current laws <br />and regulations, particularly related to managed care. <br />Task/Action: <br />(A) Provide information to eligible clients regarding their specific health care provider and insurance program. <br />3. Help low-income Santa Ana residents become more informed and active health care consumers on their own <br />behalf. <br />Task/Action: <br />(A) Explain to client what options they have, what benefits they are entitled to receive and what they can do to solve <br />their problem. <br />Schedule of Performance <br />Estimate the number of unduplicated Santa Ana residents to be served during the 12-month <br />contract period per quarter <br />Quarter l: <br />July 1 — September 30 <br />14 <br />Persons <br />Quarter 2: <br />October 1 — December 31 <br />14 <br />Persons <br />Quarter 3: <br />January 1 — March 31 <br />14 <br />Persons <br />Quarter 4: <br />April 1 — June 30 <br />16 <br />Persons <br />Total <br />58 <br />Persons <br />
