A- 2005 -192 <br />milk MEMORIALCARE' <br />ANAHEIM MEMORIAL MEDICAL CENTER <br />Agreement to Provide Service to Santa Ana Police Department for <br />Evidence Collection on Alleged Sexual Assault Victims <br />Anaheim Memorial Medical Center ( "AMMC ") and Santa Ana Police Department agree as <br />follows: <br />AMMC agrees to provide evidence from rape and sexual assault examinations in regards to <br />criminal related matters. <br />SPECIAL PROVISIONS: <br />1. Term of Agreement, July 1, 2005 through June 30, 2006. <br />2. Charge per examination, to be paid by Santa Ana Police Department $650.00. <br />3. Charge per victim follow up examination $100.00. <br />4. Charge per suspect examination $150.00. <br />5. AMMC agrees to provide Santa Ana Police Department approved medical exams to <br />alleged sexual assault and rape victims for evidence. The exam is to include <br />videotaped colposcopic examination and photographs. The colposcopic examination <br />will be conducted by medical personnel who are competent in the use of the <br />equipment and the interpretation of findings. <br />6. AMMC agrees to provide competent court testimony when required (upon receipt of <br />summons) <br />7. All evidence collected, including video tape of the colposcopic examination, will be <br />relinquished at the conclusion of that examination. Photographs will be available, <br />upon request by investigators. <br />8. AMMC agrees to make results of and evidence from examination available to <br />Santa Ana Police Department personnel at any time subsequent to the date of <br />examination upon request of summons. <br />9. By signing this Agreement, AMMC certifies that it does not discriminate in hiring or <br />treatment on the basis of race, color, creed, religion, sex, sexual orientation, age, <br />marital status, national origin, ancestry, physical handicap or medical conditions. <br />10. This Agreement is not meant to prohibit the Santa Ana Police Department from <br />utilizing the services of other hospitals. <br />Agreed to this day of June, 2005 <br />Anaheim Memorial Medical Center <br />1111 W.
