<br />1 i nc 1 udi ng i nterna 1 audi t procedures and monitori ng of subcontractor's <br />2 performance until completion of services. <br />3 Upon ADMINISTRATOR's approval of CONTRACTOR's proposed <br />4 procurement system, CONTRACTOR shall comply with such procurement system in <br />5 obtaining subcontracts with a total cost in excess of ten thousand dollars <br />6 ($10,000) during the term of this Agreement. In addition, CONTRACTOR shall <br />7 obtain ADMINISTRATOR's written consent prior to entering into a subcontract <br />8 with any organization when the total cumulative cost of services to be <br />9 provided by that organization is anticipated to exceed ten thousand dollars <br />10 ($10.000) during the term of this Agreement. <br />11 CONTRACTOR and its subcontractor (s) sha 11 estab 1 i sh and <br />12 maintain accurate and complete financial records related to services provided <br />13 under the terms of thi s Agreement. Such records may be subject to the <br />14 satisfaction of ADMINISTRATOR. and to the examination and audit by <br />15 ADMINISTRATOR or designee. for a period of four (4) years after the date of <br />16 final payment under this Agreement. or until any pending audit is completed. <br />17 B. FORM OF BUSINESS ORGANIZATION AND REAL PROPERTY DISCLOSURE <br />18 B.1 Form of Business Organization: <br />19 Upon the request of ADMINISTRATOR. CONTRACTOR shall prepare and <br />20 submit, within thirty (30) days thereafter. an affidavit executed by persons <br />21 satisfactory to ADMINISTRATOR containing, but not limited to, the following <br />22 information: <br />23 B.1.1 The form of CONTRACTOR's business organization. i.e., <br />24 proprietorship, partnership, corporation. etc. <br />25 B.1.2 A detailed statement indicating the relationship of <br />26 CONTRACTOR. by way of ownership or otherwise, to any parent organization or <br />27 individual. <br />28 B.1.3 A detailed statement indicating the relationship of <br /> <br />(WSH060S) <br /> <br />10 of 37 <br /> <br />(March 7. 200S) <br />
