<br />1 24. PERSONNEL DISCLOSURE <br />2 24.1 CONTRACTOR shall make available to ADMINISTRATOR a current list of <br />3 all personnel providing services hereunder, including resumes and job <br />4 applications. Changes to the list will be immediately provided to <br />5 ADMINISTRATOR in writing, along with a copy of a resume and/or job <br />6 application. The list shall include: <br />7 24.1.1 All full or part-time staff positions by title, including <br />8 volunteer positions whose direct services are required to provide the programs <br />9 described herein: <br />10 24.1.2 A brief description of the functions of each position and <br />11 the hours each person works each week, or for part-time personnel each day or <br />12 month, as appropriate: <br />13 24.1.3 The professional degree. if applicable, and experience <br />14 required for each positlon: and <br />15 24.1.4 The language skill, if applicable. for all personnel. <br />16 24.2 Where authorized by law, CONTRACTOR shall conduct criminal record <br />17 background checks on all employees and/or volunteers who will provide services <br />18 under this Agreement. <br />19 24.3 CONTRACTOR warrants that all persons employed or otherwise <br />20 assigned by CONTRACTOR to provide services under this Agreement have <br />21 satisfactory past work records and/or reference checks indicating their <br />22 ability to perform the required duties and accept the kind of responsibility <br />23 anticipated under this Agreement. CONTRACTOR shall maintain records of <br />24 background i nvesti gat ions and reference checks undertaken and coordi nated by <br />25 CONTRACTOR for each employee and/or volunteer assi gned to provi de servi ces <br />26 under thi s Agreement for a mi nimum of four (4) years from the date of fi na 1 <br />27 payment under thi s Agreement or unti 1 all pendi ng COUNTY, State and Federal <br />28 audits are completed. whichever is later, in compliance with all applicable <br /> <br />(WSH0605) <br /> <br />29 of 37 <br /> <br />(March 7, 2005) <br />
