5/OS <br />including: (i) notification, including distribution of the lead <br />hazazd information pamphlet (Attachment No. 1), disclosure <br />and notices of hazard evaluation and reduction (Attachments <br />No. 2 and 3); (ii) evaluation of lead hazazd(s), including paint <br />testing and risk assessment; (iii) reduction of lead hazard(s), <br />including safe work practices and clearance, and <br />confirmation of clearance; and (iv) ongoing maintenance, as <br />and if applicable for multifamily properties. <br />(ii) In this regard SUBRECII'IENT shall track information on <br />all persons/households assisted and the housing units assisted <br />with the CDBG FUNDS. Attachment No. 5 hereto is a sample <br />form for tracking persons/households and housing units <br />assisted with the CDBG FUNDS. All categories of <br />information set forth in such tracking form shall be prepazed <br />and submitted to the CITY with the April 15 quarterly report. <br />(iii) In the event that such information and reports are not <br />complete and reasonably satisfactory information submitted in <br />order for CITY staff to review and evaluate compliance with <br />applicable CDBG REGS and LBP Regs, then the final quarterly <br />disbursement payment shall be withheld from the <br />SUBRECIPIENT. <br />(iv) Documentation of clearance confirmation as provided in <br />Section L(1)(g) hereinabove. <br />(~) Reports <br />(i) Payment Request. Concurrently with the submittal of each <br />quarterly report, on or before the 15th day of October, January, <br />April and July, SUBRECIPIENT shall submit both: an original <br />invoice and true copies of invoices, receipts, agreements or <br />other documentation supporting and evidencing how the CDBG <br />FUNDS have been expended during the applicable quarter. <br />(ii) Quarterly Activity Report: Including in such reports <br />evidence of compliance with all applicable provisions of Title X <br />and the LBP Regs, in particular Subpart K and, as applicable <br />Subpart J, as further described in subsection (d) hereinafter, and <br />Subpart M, if SUBRECIPIENT provides long term tenant-based <br />rental assistance. SUBRECII'IENT agrees to provide CITY with <br />written cumulative (yeaz-to-date) reports of its activities on or <br />before the 15th day of October, January, April and July for the <br />10 <br />
