<br />.. .~ <br /> <br />A - :<(0 I-/D),-C:::-" <br /> <br />INSURANC( UN flLt <br />WORK MAY PHOCEED <br />UNTIL INSUR/\NCE EXPIRES <br />)0-15"-05 <br />CLERK OF COUNW <br />DA TE::{".-Ilf - 0 5 <br /> <br />THIRD AMENDMENT TO AGREEMENT <br /> <br />G Pb <br />lFi!;(~ <br /> <br />THIS THIRD AMENDMENT TO AGREEMENT is entered into this 12 day <br />of ~-;?nV ,2005, by and between Vet Care Vaccination Services, Inc., a California <br />corporati n ("Consultant") and the City of Santa Ana, a charter city and mumclpal <br />corporation of the State of California ("City"). <br /> <br />Recitals: <br /> <br />A The parties entered into Agreement #A-2001-102, dated May 7, 2001, (hereinafter <br />"said Agreement") by which Consultant has provided low cost dog and cat <br />vaccination clinics to serve City residents. <br /> <br />B. In accordance with the terms and conditions of said Agreement, the parties wish to <br />renew said Agreement for an additional one-year period. <br /> <br />Wherefore, in consideration of the covenants contained in said Agreement, and subject <br />to all the terms and conditions of the Agreement, except those amended in this Third <br />Amendment to Agreement, the parties agree as follows: <br /> <br />I. Section I, SCOPE OF SERVICES, shall be deleted in its entirety and replaced with <br />the following: <br /> <br />"Consultant shall conduct low cost dog and cat vaccination clinics for City residents <br />at the times, dates and locations mutually agreed on by the parties to this Agreement. <br />The parties agree the no "implied professionalism", as that term is interpreted by <br />regulatory agencies of the State of California, shall accrue to the City or any of its <br />facilities or locations, from the conduct of said vaccination clinics ("Clinics"). <br />Consultant shall perform all services at the rates and charges as set forth in Exhibit A- <br />I to this Amendment to Agreement." <br /> <br />2. Section 4, TERM, shall be amended to read as follows: <br /> <br />'This Agreement shall commence on May 7, 2001 and terminate on June 30, 2006, <br />unless terminated earlier in accordance with Section 13, below. The term of this <br />Agreement may be extended upon a writing executed by the Chief of Police and the <br />City Attorney." <br /> <br />II <br /> <br />II <br />
