<br />14.7 <br /> <br />14.8 <br /> <br />by which the Association will take over the administration <br />of this fund during the term of this Agreement. Once the <br />Association accepts responsibility for the administration <br />of the plan, the FBA will provide an independent actuarial <br />valuation to the City on an annual basis. <br /> <br />Vision Insurance. Effective as soon as practicable, the <br />City agrees to implement, through payroll deduction, a <br />non-participatory vision care plan through Eye Med. This <br />plan is voluntary on behalf of the employee and shall be <br />fully funded by the participating employee. <br /> <br />Employees on unpaid leave of more than 15 calendar days <br />shall not receive the City's contribution toward any <br />insurance coverage. The Personnel Department will notify <br />the Association of all employees on unpaid leave or who <br />separate from City employment within three (3) working <br />days. The City will continue the payment of contribution <br />for insurance coverage until the end of the month in which <br />the Association had received notice from the City of the <br />employee's separation from employment. <br /> <br />68 <br />
