Description of Work - in space below, describe the progqam to be funded <br />during the 12 -month contract period including, services t be provided, <br />program goals, client characteristics, & how grant funds ill be utilized. <br />The primary need that we will be fulfilling for the elderty residents of Santa <br />Ana in our senior program is to provide services, access to information, and <br />the critical socialization that the elderly need in order to keep them active in <br />society. <br />Goal 1: Link Seniors to health and human services. <br />Goal 2: Improve Senior health and well-being byprovii fing a forum for <br />Socialization. <br />Goal 3: Increase Seniors access to critical information and resources <br />Nearly all beneficiaries ofourprogram - especially soca l serviceprograms - <br />should be of "low/moderate-income. " Under this criteri n, the program must <br />provide benefits to Santa Ana residents whose househola incomes are not <br />more than 80% of Orange County's median income, adji rstedforfamily size. <br />The following categories are generally PRESUMED to t e low/moderate- <br />income persons: elderly (62 and older). <br />Schedule of Performance <br />Estimate the amount of grant funds to be requested during the 12-m nth contract period <br />on the quarterly bases: <br />Quarter 1: July 1 - September 30 5 -Persons <br />Quarter 2: October 1 - December 31 5 Persons <br />Quarter 3: January I - March 31 5 Persons <br />Quarter 4: April 1 - June 30 10 Persons <br />TOTAL 25 Pe sons <br />Invoicin¢ Schedule <br />Estimate the amount of grant funds to be requested during the 12-m nth contract period <br />on a quarterly basis: <br />Quarter 1: July 1 - September 30 $ 1.500. <br />Quarter 2: October I - December 31 $ 1,500. <br />Quarter 3: January 1 - March 31 $1,500, <br />Quarter 4: April 1 -June 30 $ 500. <br />TOTAL $ 5,000. <br />EXHIBIT A <br />p.5 <br />
