All written reports, information, data, charts, tables, & maps in electronic form. <br /> <br />Prior to commencement of work, the consultant shall submit samples of the radar <br />speed survey field sheet, the radar speed survey analysis sheet, the field review <br />summary sheet and the Summary of Recommendations chart for preapproval. <br /> <br />III. PROJECT REQUIREMENTS <br /> <br />A. Proposal Submittal <br /> <br />Six (6) copies of the proposal are due in the office ofthe Executive Director, Public <br />Works Agency, 20 Civic Center Plaza, M-21, Fourth Floor, Santa Ana, CA 92701 <br />no later than 5:00 p.m., on Monday, October 24, 2005. NO PROPOSAL WILL <br />BE ACCEPTED AFTER THE DUE DATE AND TlME. <br /> <br />The fee schedule (one copy) must be separately bound, sealed and submitted to <br />the City with the proposals. To be considered, all proposals shall be completely <br />responsive to the RFP. Provide a synopsis offirm's history. <br /> <br />2. Identify the project manager and key personnel assigned to the project. <br />Describe their qualifications, education and representative experience <br />particularly related to this type of project. <br /> <br />3. Identify each person who will conduct radar speed surveys and provide <br />certification by the manufacturer of the radar gun used for the survey. <br /> <br />4. Provide a brief description of representative work involving projects <br />which are similar in scope to the current project. <br /> <br />3 <br />
