IS <br />B--,,~ -0' ( (1ft/' <br />AMENDMENT TO AGREEMENT FOR THE PROVISION OF SERVICES <br />BY EAST EDINGER MEDICAL CENTER <br /> <br />THIS AMENDMENT TO AGREEMENT (AMENDMENT) made and entered into <br />this VAl.! day of t7~J_~ 2001, by and between the City of Santa <br />Ana, a charter city and unicipal corporation of the State of California ("CITY") and East <br />Edinger Medical Clinic (VENDOR'). <br /> <br />Recitals: <br /> <br />WITNESSETH <br /> <br />A. CITY AND VENDOR entered into an Agreement for the Provision of <br />Services by East Edinger Medical Clinic, dated September 24, 2000, by which <br />VENDOR agreed to perform medical screening services, influenza vaccines and <br />wellness physicals (the "AGREEMENr). <br /> <br />B. Pursuant to the AGREEMENT, the time for completion of the services was <br />June 30, 2001. The parties now desire to extend the time for completion to June 30, <br />2002. <br /> <br />WHEREFORE, in consideration of their mutual and respective promises, and <br />subject to the terms and conditions of the AGREEMENT as hereby amended, the <br />parties hereby agree as follows: <br /> <br />1. Section 1 of the AGREEMENT is hereby amended to read as follows: <br /> <br />'The services to be performed pursuant to this Agreement shall be <br />completed on or before June 30, 2002, unless extended in a writing <br />executed by the Executive Director of the Personnel Agency and the <br />City Attorney." <br /> <br />2.
