VI. PROPOSAL FORM <br />Responses to this Request for Proposal are due at 5:00 p.m. WEDNESDAY, NOVEMBER 24, 2004. <br />The undersigned Vendor agrees to provide carnival services at City-owned parks in accordance with <br />the specifications. I/We have stated herein the services and fees that I/we will famish and deliver as <br />specified. <br />Award shall be based upon the evaluation criteria included in Section III.
