<br />'-' ...., <br />GOALS AND OBJECTIVES <br />IMPLEMENTATION STRATEGY <br /> <br />Goal: To empower students to make healthy lifestyle choices by increasing their awareness, <br />knowledge and ability to practice important life skills (such as decision making, goal-setting, <br />identifying values, communicating effectively and others). <br /> <br />.Objectives: <br />I. To prQvide the Smart Choices program to 200 at-risk youth from Santa Anll CDBG <br />eligible middle schools. <br />Task/Action <br />* Meet with potential participating sites to discuss program. <br />* Meet with site counselors and teachers to discuss program details and identify youth who will <br />most benefit by participating in the program. <br />* Recruit targeted youth to participate in the program. <br />* Orient the youth and their parents to program structure answer questions, and obtain parent <br />permission. <br />* Obtain and file the registration forms for further use. <br /> <br />2. Provide 16-20 hours of instruction in the Smart Choices program. <br />Task/Action <br />* Tailor the curriculum to best meet recipient criteria <br />* Administer a satisfaction survey to the school staff, at the end of the program. <br />* File the surveys for further use. <br />* Orient program instructors on implementation and maintenance of daily class rosters. <br />* File the class rosters for further use. <br /> <br />3: At the conclusion of the program: <br />. 60 -70% of the participants will be able to demonstrate skill improvement in the areas of <br />personal growth, life management and service learning as a result of this program. <br /> <br />. 60 -70% of the participants will be able to demonstrate an improved attitude to their <br />future and prospects as a result of this program. <br /> <br />. 60 -70% of the participants will be able to identify healthy behaviors versus risk- <br />taking/unhealthy behaviors and their short- and long-term consequences. <br /> <br />Task/Action <br />* All of the above objectives will be fulfilled by establishing a curriculum that includes <br />enrichment components such as personal growth, life management and risk-taking prevention. <br />* Assign qualified staff to implement this program. <br />* Administer pre-test to the students, at the beginning of the program. <br />* Administer post-test to the students at the conclusion of the program. <br />* Administer the satisfaction survey to the school staff. <br />* Record, pool, analyze and present the results of the quantitative and qualitative evaluation data. <br /> <br />Pi!I:!f l& <br />[XHIBI"I t"I <br />
