Provide <br />projected numbers of Santa Ana residents to be served, and the kinds of services they would <br />receive if your program is funded. Also describe the specific actions and tasks that-would be <br />performed on a day-to-day basis to achieve these goals. <br /> <br />See Appendix A for examples. <br /> <br />Goal: To help low-income people access quality health services and navigate the delivery <br />systems as well as empowering consumers to be active participants in their own healthcare and <br />in planing programs designed to serve them. Although the HCAC provides outreach services in <br />addition to direct client services. LASOC is requesting funding to assist with direct services. The <br />following objectives relate to the direct service component of HCAC. <br /> <br />Obiective(s): <br />1. Help 50 low-income residents of Santa Ana resolve problems related to access to <br />healthcare services or programs. <br />Task/Action: <br />(A) Screen applicant for eligibility through the hotline Monday through Friday and <br />provide advice and counsel to eligible clients. <br />(B) Represent clients who need in-depth services. <br />2. Help low-income Santa Ana Residents become more infomned and educated about <br />their rights under current laws and regulations, particularly related to managed care. <br />Task/Action: <br />(A) Provide information to eligible clients regarding their specific health care provider <br />and insurance program. <br />3. Help low-income Santa Ana residents become more infomned and active health care <br />consumers on their own behalf. <br />Task/Action: <br />(A) Explain to client what options they have, what benefits they are entitled to receive <br />and what they can do to solve their problem. <br /> <br />C) <br /> <br />c) <br /> <br />F;tHfl3ll fl <br />
