To provide shelter and program services for up to 120 low-income, ethnically diverse families, couples <br />and single women per night in both programs, with 60-65 individuals within the Transitional Program, <br />. Provide 19,000 bed nights to 25 families annually in the Transitional Program, <br />. Provide 30,000 meals annually in the Transitional Program, <br />. Of the above mentioned numbers, 20 of the residents will be from Santa Ana, <br /> <br />Objectives: <br />(1) Accept up to 18 homeless families at a time, including annual total of 20 Santa Ana <br />residents, in the Transitional Program. <br />Task/Action: <br />(a) Make contact with service organizations within Orange County to ensure they are <br />acquainted with the services ofOCIS. <br />(b) Accept referrals and phone calls for beds available daily for Emergency Program. <br />(c) Interview families in the Emergency Program for entrance into the Transitional <br />Program. <br /> <br />(2) Provide for basic needs of food, shelter and clothing. <br />Task/Action: <br />(a) Maintain supplies needed by both emergency and transitional clients, <br />(b) Ask various organizations in the community to have drives for needed items <br />throughout the year. <br />(c) Maintain an inventory of school supplies, uniforms, and back packs for children <br />when they enroll in school. <br /> <br />(3) Provide case management services to homeless families. <br />Task/Action: <br />(a) Emergency Program Advocate interview clients and complete pre-intake for the <br />Transitional Program, then Family Advisor interview potential families for <br />Transitional Program and complete intake process. <br />(b) Discuss potential families at Case Management meetings with all staff and <br />determine potential for successful completion of program. <br />(c) Develop case plan with families as they enter OCIS Transitional Program and <br />weekly while participating in the program.
