<br />"'-' <br /> <br />v <br /> <br />3. Contractor shall establish at least one goal for each participant as <br />related to the elements identified in their ISS. If the participant is <br />deficient in basic reading and/or math skills, one primary goal must <br />be for basic skills and must be the first service provided. Goals are <br />to be based on appropriate age standards and entered in <br />participant's ISS. <br /> <br />4. Contractor shall provide an enrollment orientation for each <br />participant which at the minimum includes the following information: <br /> <br />a. Program goals and objectives <br />b. Length of program period <br />c. Complaint handling procedures <br />d. Available supportive services <br />e. Anticipated exit date <br />f. Hands-on orientation at the Santa Ana WORK Center <br /> <br />Participant orientation shall be documented and placed in <br />participant's file. An orientation of the Santa Ana W/O/RlK Center <br />wili also be provided. <br /> <br />5. Contractor shall provide participant services in accordance with the <br />program schedule and curriculum attached hereto as the program <br />narrative (Exhibit A). Neither the program schedule nor curriculum <br />may be altered in any way during the term of this agreement, <br />without prior written approval given to contractor by the Santa Ana <br />Workforce Investment Board (WIB). <br /> <br />6. Contractor shall maintain daily, weekly, and monthly classroom and <br />internship training time and attendance records for all participants. <br />Copies of attendance records shall be placed in participant's file <br />upon completion of program. <br /> <br />7. Contractor shall maintain individualized counseling records and <br />shall make these records available for review and inspection upon <br />request by City staff. All such records shall remain otherwise <br />confidential and shall be kept in a form approved by City. <br /> <br />8. Contractor shall maintain and share with other Youth partners the <br />Individual Service Strategy (ISS) for each participant from time of <br />enrollment through and including twelve (12) months after he/she is <br />exited from the program. <br /> <br />9. Full completion of the program and its elements shall be defined as <br />and determined by individual participant attainment of the learning <br />objectives and demonstrated achievement of the standards for <br />
