GOALS AND OBJECTIVESIIMPLEMENTATION STRATEGY <br /> <br />Goal: To graduate 30 families (120 individuals) into self sufficiency and transition them to <br />independent living in a permanent residence. Of this number 6 families (30 individuals) are <br />planned to be former Santa Ana residents. <br />Obiectives: <br />1. Supply food and shelter to 30 families during FY2002/03. <br />Task/Action: <br />(A) Have daily interface with other shelters, social service organizations, former clients <br />and our own local phons: hot line to determine clients with the most pressing needs. <br />(B) Interview prospective clients and generate a "get well" program with them. <br />(C) Provide accomodations for the clients (Esoivtated to be 18,00 bed-days for <br />FY2002/03). <br />(D) Provide the accomodations at a low cost benefit ratio. <br />2. Supply life skills services to all clients at the shelter by various organizations to aid in <br />providing self-sufficiency and the transition to independent living. <br />Task/Action: Life Skill Service Provided by <br />Individual & Family Counseling -Lutheran Social Services <br />Budgeting - In-house staff <br />Child Mentors -Bridge learning center <br />Parenting and Nutrition -Lutheran Social Services <br />Job Development & Computer course -In-House Staff <br />English as Second Language -Volunteers <br />Student Homework Assistance -Volunteers <br />Arts and Crafts -Fountain Valley High School <br />Medical and Welfare Assistance -Thomas House Staffguidamce <br />Drug Prevention -La Familia Counseling Center <br />Transportation Services -Thomas House <br />Clothing Assistance - Thomas House & Good Will <br />Well Baby Care .Public Health Nurses <br />Child Care -Licensed Day Care <br />Client Oureach Services -Thomas House <br />3. Prepare client for after shelter independent living. <br />Task/Action: (A) Have client save 80% of all earnings while at the shelter. <br />(B) Have client write weekly goals in conjunction with the case worker and <br />then track performance weekly with the case worker. <br />(C) Assist client in finding permanent housing. <br />(D) Conduct an exit interview when the client leaves the shelter. <br />(E}R.(lflder after-exit assistance to the clients when possible. <br /> <br />PI\I5~ 7A <br />EXffl8 1'/ A- <br />
