Attachment A <br />FY05 HOMELAND SECURITY GRANT PROGRAM <br />2. DUNS Number. The SAA must provide a Dun and Bradstreet (D &B) Data Universal <br />Numbering System (DUNS) number with the FY05 HSGP application. An <br />application will not be considered complete until a valid DUNS number is <br />provided by the applicant_ This number may be provided in one of the attachment <br />fields within the online GMS application. Organizations should verify having a DUNS <br />number or take the steps necessary to obtain one as soon as possible. Applicants <br />can receive a DUNS number at no cost by calling the dedicated toll -free DUNS <br />Number request line at 1- 800 - 333 -0505. <br />3. Program Narrative. Grantees must build and maintain the capabilities necessary to <br />effectively establish and sustain the many programs critical to achieving states' <br />homeland security strategies. To help assess these capabilities, the FY05 HSGP <br />requires that all award recipients submit a narrative as a part of the HSGP <br />application. This narrative must provide a general overview of support systems and <br />processes planned or in place within the states to administer homeland security <br />programs. To frame the narrative, please use the following structure: <br />PART I: Management Capabilities. Please provide a description of current <br />resource management capabilities to develop, implement, and manage the <br />programmatic requirements associated with HSGP funding and other federally <br />funded programs supporting the State Homeland Security Strategy. When <br />describing current management capabilities, consider including such information <br />as the following: <br />• Identify the major organizations, committees, and other structures <br />accountable for implementing the homeland security strategy and programs <br />• Provide an overview of the governance process and operating guidelines to <br />administer and ensure alignment of major federally funded programs <br />• Indicate the staffing /resource levels within those core organizations <br />responsible for program administration and oversight <br />• Identify mechanisms used for statewide coordination among HSGP funds and <br />other programs <br />• Provide an overview of the process(es) used to determine funds allocation <br />• Identify systems /processes used to facilitate fund disbursement <br />• Describe the state's funds monitoring and tracking system(s) for major <br />programs. <br />In addition, the description must make specific reference to the following: <br />• Current efforts to achieve National Incident Management System (NIMS) <br />implementation and institutionalization across the entire state /territory <br />response system during FY05 <br />• The existence of or current efforts to establish a senior advisory committee <br />(see Senior Advisory Committee on page 22 for additional guidance) during <br />FY05 to coordinate HSGP funding as well as with the U.S. Department of <br />Health and Human Services' (HHS) Centers for Disease Control and <br />DEPARTMENT OF HOMELAND SECURITY I OFFICE FOR DOMESTIC PREPAREDNESS <br />12 <br />i <br />a <br />I J <br />a <br />
