Direct the City Attorney to prepare and authorize the City Manager <br />and Clerk of the council to execute an amendment to agreement A- <br />2005-143-00 with U.S. Healthworks, for an additional year, in an <br />additional amount of $9B,000, for a total aggregate amount not to <br />exceed $150,000. <br />2. Direct the City Attorney to prepare and authorize the City Manager <br />and Clerk of the council to execute an amendment to agreement A- <br />2005-144-00 with Westcliff Medical Laboratories, for an additional <br />year, in an additional amount of $42,000, for a total aggregate <br />amount not to exceed $77,000. <br />3. Direct the City Attorney to prepare and authorize the City Manager <br />and Clerk of the council to execute an amendment to agreement A- <br />2005-145-00 with Santa Ana College, for an additional year, in an <br />additional amount of $15,000, for a total aggregate amount not to <br />exceed $27,000. <br /> <br />DISCUSSION <br /> <br />The City requires annual comprehensive physical evaluations for the Fire <br />Department's sworn personnel. The Police Department offers the service, <br />on a voluntary basis, to all personnel also. The program provides one- <br />on-one feedback, including recommended programs for physical <br />improvements, methods to assist in the early detection and treatment of <br />illnesses or injuries and to assist in minimizing time lost from them. <br />In addition the Personnel Services Agency uses U.S. Healthcare to provide <br />"risk exposure" physicals and low cost flu vaccinations to City <br />employees. <br /> <br />To provide the best service <br />April 2005 (RFP 05-052). <br />awarded by Council in June <br />the three agreements for <br />aggregate amounts. <br /> <br />possible a Request for Proposal was issued in <br />Based on that RFP, three agreements were <br />2005.
