<br />City of Santa Ana <br />Emergency Shelter Grant <br />Scope of Work <br />Name of Organization- Mental Health Association of Orange County <br />Name of Funded Program- Santa Ana Outreach Program <br />Annual Accomplishment Goal <br />I. Total number of un duplicated clients (Santa Ana and Non-Santa Ana Residents) anticipated to be served by <br />the funded program, named above, during the 12-month contract period. <br />2000 PERSONS <br /> <br />II. Number of only unduplicated Santa Ana residents to be served by the funded program, named above, <br />during the 12-month contract period. <br />600 PERSONS <br /> <br />Program and Funding Description <br /> <br />III. Description of Work - In the space below, describe the program to be funded during the 12-month <br /> <br />contract period. <br />Under direction of the Outreach Program Manager, the Outreach Worker is responsible for providing outreach services to homeless <br />mentally ill adults with-in the Santa Ana area, with a focus on establishing linkages between the homeless mentally ill and a variety <br />of services including shelter, social services, medical and psychiatric care, and MHA's Multi-Service Center as well as the <br />continuum of services provided thoughout the County of Orange. The Outreach Workers engage the Santa Ana homeless individuals <br />in a variety of settings such as parks, food programs, shelters, etc., to assess their need for services. They further assist homeless <br />persons with transportation to medical services, mental health clinics, shelter beds, social service agencies, and offer access to <br />MHA's Multi-Service Center. The Outreach Worker conducts initial assessments in the field to determine program eligibility and <br />identify entitlements and resources that the individual may require and for which they may be eligible. MHA maintains statistics and <br />other program paperwork in accordance with Agency (MHA), The City of Santa Ana and HUD requirements. Staff attend regularly <br />scheduled supervision and city/county meetings to futher enable them in providing crisis intervention services to address acute <br />psychiatric episodes, health emergencies, or personal crisis. The Outreach Worker for Santa Ana is bi-lingual Spanish to assist clients <br />with cultural, social and communication skills at a level sufficient to engage consumers, to build trusting relationships, and to <br />encourage them to participate in the program. All MHA Staff have an excellent knowledge of entitlement programs, housing and <br />shelter resources, legal services, mental health and substance abuse services, health care resources, vocational training and <br />rehabilitation, and employment assistance. <br /> <br />IV. Activities funded by this grant - How will ESG funds be used for the funded program? Narratively <br />describe the budget for the funded program illustrated in Exhibit B and B-1. <br />The funds allocated will be used for milage, salaries, trainings for staff and essentials needed by the mentally ill homeless Santa Ana <br />individuals we serve. <br /> <br />Exhibit A <br />Page 1 of2 <br />
