Total number of un duplicated clients (Santa Ana and Non-Santa Ana Residents) anticipated to be served by <br />the funded program, named above, during the l2-month contract period. <br />i2..PERSONS <br /> <br />I1. Number of only unduplicated Santa Ana residents to be served by the funded program, named above, <br />during the l2-month contract period. <br />i2..PERSONS <br /> <br />Program and Funding Description <br />IlL Description of Work - In the space below, describe the program to be funded during the l2-month <br />contract period. <br />Provide a mentor program for high school seniors from the high schools in the City of Santa Ana for the program year 2006-2007. <br />We will match students will adult mentors for this crucial year of schooling. We will be in addition hosting II night meetings where <br />we have individuals and company from the community make presentations to the students and mentors. Weekly at lunch the students <br />meet with staff to go over scholarships, fmancial aid, collerge applications. Are primary goal is make sure that the students graduate <br />and make a choice in a post-secondary education and become contributing members of society. <br /> <br />IV. Activities funded by this grant - How will CDBG funds be used for the funded program? Narratively <br />describe the budget for the funded program illustrated in Exhibit B and B-1. <br />The funds provided by COHO will be used to help in salaries of the two paid staff members. The budget shows the percentages and <br />amounts that will be used in each category. The staff members are responsible for recruitment, program development, and record <br />keeping for both studnets and mentors. <br /> <br />Exhibit A <br />Page 1 of2 <br />
