City of Santa Ana <br />Emergency Shelter Grant <br />Scope of Work <br />Name of Organization- Laura's House <br />Name of Funded Program- Domestic Violence Emergency Shelter & Supportive Services Program <br />Annual Accomplishment Goal <br />I. Total number of unduplicated clients (Santa Ana and Non -Santa Ana Residents) anticipated to be served by <br />the funded program, named above, during the 12 -month contract period. <br />175 PERSONS <br />II. Number of only unduplicated Santa Ana residents to be served by the funded program, named above, <br />during the 12 -month contract period. <br />12 Sheltered PERSONS <br />Program and Funding Description <br />III. Description of Work — In the space below, describe the program to be funded during the 12 -month <br />contract period. <br />Laura's House will provide a comprehensive 30 -45 day residential shelter program to 12 battered women and children who are <br />residents of Santa Ana and are homeless as a result of domestic violence. The emergency shelter program includes food and <br />clothing; intensive groups (6 -8 per week) & individual counseling (1 -2 times /week); case management; parenting; life skills and <br />domestic violence education classes; therapeutic childrens programs; legal advocacy & assistance and social advocacy with resources <br />necessary for the client to transition from shelter living to independence and self - sufficiency . <br />IV. Activities funded by this grant — How will ESG funds be used for the funded program? Narratively <br />describe the budget for the funded program illustrated in Exhibit B and B -1. <br />As identified above we estimate that during the 2006 -2007 contract year a minimum of 7% of clients served in our Emergency <br />Shelter will be residents of Santa Ana. Laura's House will use the ESG funds provided (2.3% of program budget) to provide a full <br />range of services in our Emergensy Shelter to low- income residents of Santa Ana who are homeless due to domestic violence. <br />Shelter Manager administration salary support, this position oversees all staffing and operations for the Emergency Shelter. <br />Hotline Coordinator /Intake Case Manager salary support, this position completes all screening for shelter residency and completes <br />the intake process with the client upon entry into the shelter. <br />Clinical Case Manager salary support, provides intensive case management, linkage with financial, housing and employment <br />resources in addition to assessment of clinical needs to be addressed in order to move the clients to self - sufficiency. <br />Shelter Staff salary support, provides crisis intervention, support, life skills mentoring and assistance to shelter clients. <br />Shelter Operation costs such as communication for our phone and computer systems, maintenance & repairs to the facility, <br />equipment and/or transportation van for clients, and insurance. <br />Exhibit A <br />Page 1 of 2 <br />
