Total number of unduplicated clients (Santa Ana and Non -Santa Ana Residents) anticipated to be served by <br />the funded program, named above, during the 12 -month contract period. <br />1224PERSONS <br />II. Number of only unduplicated Santa Ana residents to be served by the funded program, named above, <br />during the 12 -month contract period. <br />90OPERSONS <br />Program and Funding Description <br />III. Description of Work — In the space below, describe the program to be funded during the 12 -month <br />contract period. <br />The Mercy House Center is an outreach center which addresses emregency needs for the homeless of Orange County. The Center <br />provides a variety of services such as: bus passes and hygiene supplies. <br />Through our Building Bridges Program, the goal of the Center is to improve the ability of clients to obtain services, employment, and <br />housing opportunities. Additionally, we will increase their knowledge of the community resources and connect them appropriately. <br />The Center will provide clients with a birth certificate, driver's license, or IDs. Also, clients will be provided with access to the <br />telephone and referrals. <br />Through our Basic Needs Program, the goal of the Center is to improve the living conditions of the clients, increase their access to <br />transportation, and improve their health conditions. The Center will provide clients with hygiene supplies, clothing and food <br />vouchers, transportation assistance, and medication. <br />IV. Activities funded by this grant — How will ESG funds be used for the funded program? Narratively <br />describe the budget for the funded program illustrated in Exhibit B and B -1. <br />Mercy House will be using ESG funds to cover costs associated with staff salaries, office supplies, rent, insurance, and client <br />services, <br />Exhibit A <br />Page 1 of 2 <br />
