City of Santa Ana Emergency Shelter Grant Scope of Work Name of Organization- Mercy House Transitional Living Centers, Inc. Name of Funded Program- Mercy House Center Annual Accomplishment Goal I. Total number of unduplicated clients (Santa Ana and Non -Santa Ana Residents) anticipated to be served by the funded program, named above, during the 12 -month contract period. 1224PERSONS II. Number of only unduplicated Santa Ana residents to be served by the funded program, named above, during the 12 -month contract period. 90OPERSONS Program and Funding Description III. Description of Work — In the space below, describe the program to be funded during the 12 -month contract period. The Mercy House Center is an outreach center which addresses emregency needs for the homeless of Orange County. The Center provides a variety of services such as: bus passes and hygiene supplies. Through our Building Bridges Program, the goal of the Center is to improve the ability of clients to obtain services, employment, and housing opportunities. Additionally, we will increase their knowledge of the community resources and connect them appropriately. The Center will provide clients with a birth certificate, driver's license, or IDs. Also, clients will be provided with access to the telephone and referrals. Through our Basic Needs Program, the goal of the Center is to improve the living conditions of the clients, increase their access to transportation, and improve their health conditions. The Center will provide clients with hygiene supplies, clothing and food vouchers, transportation assistance, and medication. IV. Activities funded by this grant — How will ESG funds be used for the funded program? Narratively describe the budget for the funded program illustrated in Exhibit B and B -1. Mercy House will be using ESG funds to cover costs associated with staff salaries, office supplies, rent, insurance, and client services, Exhibit A Page 1 of 2
