Description of Outcome Tracking System: <br />Please describe your organization's outcome tracking system that will enable you to fulfill the outcome selected above. <br />To achieve compliance with HUD Performance Outcome Tracking guidelines, the Emergency Shelter progam must count number of <br />homeless persons given overnight shelter, with a goal of 50% of individuals transitioning to interim housing. <br />Friendship Shelter is a pilot agency in the recently implemented Orange County Client Management Information System (CMIS). <br />CMIS is a tool to help homeless service providors and others with client tracking, case management, goals measurement and reporting <br />It is designed to assist agencies and local jurisdictions to comply with HUD requirements. <br />Each client of Friendship Shelter is entered into the CMIS database upon acceptance into the facility. Demographic and case <br />management information is input as necessary during the clients stay, as is exit data including type of housing found, if known. <br />Period reports, monthly, quarterly and yearly will show the number of individuals served and interim housing achieved. <br />Exhibit A-1 <br />Page 3 of 4 <br />
