City of Santa Ana <br />Community Development Block Grant <br />Scope of Work <br />Name of Organization- Women's Transitional Living Center, Inc. (WTLQ <br />Name of Funded Program- Independence from Dependence <br />Annual Accomplishment Goal <br />L Total number of unduplicated clients (Santa Ana and Non -Santa Ana Residents) anticipated to be served by <br />the funded program, named above, during the 12 -month contract period. <br />2,500 PERSONS <br />II. Number of only unduplicated Santa Ana residents to be served by the funded program, named above, <br />during the 12 -month contract period. <br />120 PERSONS <br />Program and Funding Description <br />III. Description of Work — In the space below, describe the program to be funded during the 12 -month <br />contract period. <br />The "Independence from Dependence" program is a supportive services program designed to promote independence through <br />becoming safe, becoming addiction -free, acquiring permanent housing and/or getting ajob. The program will work with at least 120 <br />residents of Santa Ana, providing at least 5,600 hours of supportive services for adults and another 6,000 hours of services for <br />children. Outcomes will be mental health stability, decrease in addiction, increase in independence, and linkage to other supportive <br />services and housing. Services provided will include: shelter, food, clothing, case management, legal/immigration advocacy, social <br />service assistance, counseling, educational classes, 24/7 hotline, transportation, translation, substance abuse treatment, crisis <br />counseling, referral/resource assistance, a therapeutic children's program, gang intervention program, ESL /Advanced English classes, <br />process /empowerment groups, medical screening/care, job training, work experience, career counseling, job placement, and <br />housing/financial advocacy. <br />IV. Activities funded by this grant — How will CDBG funds be used for the funded program? Narratively <br />describe the budget for the funded program illustrated in Exhibit B and B -1. <br />CDBG funds will be used to partially pay the salary of a Bilingual Child Advocate who is responsible for children's <br />activities /education in the Children's Program. This advocate performs developmental assessments (in conjunction with a counselor), <br />tests the children for emotional symptoms that may interfere in the child's ability to perform in school, creates daily activities for the <br />children, enrolls children in local schools, transports children as necessary, and is responsible for all information entered into the <br />database for activites that children partake in. <br />Exhibit A <br />Page 1 of 2 <br />
