Description of Work — In the space below, describe the program to be funded during the 12-month
contract period.
Orange County On Track's award -winning Kids On Track... Through Mentoring program has transformed the lives of thousands of
children over the past 10 years. Our success has been documented by a nationally acclaimed, science -based evaluation firm: EMT
Associates.
The primary goal of the Santa Ana On Track youth program is to continue the successful life -changing mentoring program for
disadvantaged families in the Weed & Seed area. In order to combat drug use, gang involvement and educational failure, On Track
provides multi -dimensional services and opportunities that include a 4-hour weekly after -school program, positive role models,
parent education seminars, private tutoring, mentoring, and the option to participate in community service projects as well as fun
special events.
The On Track program keeps kids on track by providing vulnerable children with a structured and nurturing environment during the
critical hours of 2:30 to 6:30 p.m., where they learn and interact with trained teen mentors and On Track staff members. The On
Track teens, who have been highly screened and trained, serve as positive role models as they assist in presenting lessons on social
and character development, the joys of reading, and conflict mediation, in addition to participating in recreational activities.
Additionally, the children and their families benefit from free, fun special events throughout the year.
IV. Activities funded by this grant — How will CDBG funds be used for the funded program? Narratively
describe the budget for the funded program illustrated in Exhibit B and 13-1.
The $8,000 of CDBG funds represent 11% of the project budget. The funds will be used to help pay for the program staffs time.
Exhibit A
