Total number ofunduplicated clients (Santa Ana and Non-Santa Ana Residents) anticipated to be served by <br />the funded program, named above, during the 12-month contract period. <br />12-PERSONS <br /> <br />II. Number of only unduplicated Santa Ana residents to be served by the funded program, named above, <br />during the 12-month contract period. <br />~PERSONS <br /> <br />Program and Funding Description <br />III. Description of Work - In the space below, describe the program to be funded during the 12-month <br />contract period. <br />WISEPlace's "Steps to Independence" program is a transitional shelter program for homeless women, ages 18 to senior adults. <br />Through the "Steps" program, the residents receive safe, affordable housing, Case Management services, assistance in finding a job, <br />psychological counseling, food, clothing and access to an array of support services. The women can stay in the program for up to 12 <br />months. During their time in the program, each resident meets weekly with her Case Manager and Financial Empowerment Manager <br />to report on her progress in meetings specific goals that she has determined.. <br /> <br />IV. Activities funded by this grant - How will ESG funds be used for the funded program? Narratively <br />describe the budget for the funded program illustrated in Exhibit B and B-1. <br />Our 2006-2007 ESG grant will be applied to the salary of our Case Manager, the costs of our Security Gurad, office supplies, <br />professional services, insurance, equipment rental, utilities, food, repairs and maintenance and communications. <br /> <br />Exhibit A <br />Page 1 of2 <br />
