Total number of un duplicated clients (Santa Ana and Non-Santa Ana Residents) anticipated to be served by <br />the funded program, named above, during the 12-month contract period. <br />750PERSONS <br /> <br />II. Number of only unduplicated Santa Ana residents to be served by the funded program, named above, <br />during the 12-month contract period. <br />150PERSONS <br /> <br />Program and Funding Description <br />III. Description of Work - In the space below, describe the program to be funded during the 12-month <br />contract period. <br />.Dayle McIntosh Center's Housing program provides housing services for home seekers with disabilities, who reside in, or wish to <br />move to Santa Ana. Assistance will include maintenance of a registry of current housing listings, provision of information <br />concerning housing-related topics such as rental subsidies and home modifications, referrals of potential housing options, placement <br />and follow-up. <br /> <br />The housing coordinator can provide additional services, arranges for assistance through another staff member, or refers the <br />consumer to another agency where services are available. Individuals often need help in applying for or appealing requests for, <br />public benefits such as SSI or SSDI. Some seek instruction in daily living skills including budgeting and money management, <br />housekeeping, safety procedures, use of public transportation, etc. Most require assistance to locate permanent housing. Those <br />consumers, for which employment is a feasible option, are referred to the vocational services component for job referral. Supportive <br />services are also offered to persons with disabilities residing at emergency shelters or transitional housing facilities in the <br />community. <br /> <br />IV. Activities funded by this grant - How will CDBG funds be used for the funded program? Narratively <br />describe the budget for the funded program illustrated in Exhibit B and B-1. <br />Santa Ana CDBG Funds will be used to fund 19% FTE of the annual compensation for a Housing Coordinator position <br /> <br />Exhibit A <br />Page 1 of2 <br />
