<br />Description of Outcome Tracking System: <br /> <br />Please describe your organization's outcome tracking system that will enable you to fulfill the outcome selected above. <br />Program evaluation is determined through several methods. Performance in each service area is tracked monthly and actual <br />accomplishments compared to projected service levels. The effectiveness of service delivery is gauged in part by the degree to which <br />participating consumers reach the stated goals in their independent living plans. <br /> <br />Project effectiveness will be measured through the attainment of performance objectives. The Dayle McIntosh Center will track and <br />report the number of individuals served/trained, the amount of service provided, and the impact of the intervention on the participants. <br />The following is a summary of objectives, outputs, data to be collected, and success indicators. Goal: To work with Santa Ana <br />residents with disabilities, who require assistance to locate safe and adequate housing <br />Objective: Completion of client intake process with 25 Santa Ana residents with disabilities. <br />Activities: Develop and circulate program flyer; participate in community events and coalitions, contact area organizations and <br />agencies and establish referral system, conduct initial interview with consumers, develop and initiate individualized service plans. <br /> <br />Goal: To identify housing options for people with disabilities. <br />Objective: Achievement of housing placements for 15 Santa Ana residents with disabilities and 125 information and referrals. <br />Activities: Establish working relationships with 50 area housing providers including apartment owners/managers, low-rent motel <br />clerks, board and care operators, and individuals with rooms to rent; add 80 resources to the housing registry; acquire and post current <br />listings; make 125 referrals to consumers; assist clients with the completion of rental applications; conduct follow up to determine if <br />placement has occurred. <br /> <br />Goal: To prepare residents of Santa Ana with disabilities to successfully address specific housing issues; <br />Objective: 75% of the housing service goals of Santa Ana residents will be reached. <br />Activities: Develop fact sheet on all state and federal standards relevant to structural accessibility in housing; maintain list of licensed <br />contractors who make modifications; conduct site reviews; provide information on rental subsidy programs such as Section 8; <br />investigate and/or report alleged instances of housing discrimination; identify and refer consumers for needed fmancial aid; provide <br />500 hours of one-to-one consultation with consumers. <br /> <br />An individualized independent living Plan, (ILP), will structure the provision of services for each participant. In order to <br />remain in the program clients will be required to work toward the fulfillment of their individual plans. Progress will be summarized in <br />case files. Program effectiveness will be determined, in part, by the housing goals set and met by consumers. <br /> <br />Exhibit A-I <br />Page 3 of 4 <br />
