City of Santa Ana <br />Community Development Block Grant <br />Scope of Work <br />Name of Organization- Feedback Foundation, Inc. <br />Name of Funded Program- Senior Lunch Program <br />Annual Accomplishment Goal <br />I. Total number of unduplicated clients (Santa Ana and Non-Santa Ana Residents) anticipated to be served by <br />the funded program, named above, during the 12-month contract period. <br />1300 PERSONS <br />II. Number of only unduplicated Santa Ana residents to be served by the funded program, named above, <br />during the 12-month contract period. <br />880 PERSONS <br />Program and Funding Description <br />III. Description of Work - In the space below, describe the program to be funded during the 12-month <br />contract period. <br />The Senior Lunch program addresses the need for older adults to have a nutritional meal. However, the program is always more than <br />just a meal. Many isolated Santa Ana older adults have the opportunity, through this program, to socialize, access other services, and <br />enjoy the company of their peers, in a safe and friendly environment. For some, this is the only socialization they experience during <br />the day. A donation is suggested, but no one is ever turned away due to an inability to donate. Goal -Feedback Foundation proposes <br />to serve 880 unduplicated Santa Ana seniors during 2006/07. Feedback Foundation anticipates serving 31,000 meals at the Santa <br />Ana Lunch sites which include; Santa Ana Senior Center, Southwest Senior Center, and the Vietnamese Community Center. <br />IV. Activities funded by this grant -How will CDBG funds be used for the funded program? Narratively <br />describe the budget for the funded program illustrated in Exhibit B and B-1. <br />"The primary source of funding for these programs is the Older Americans Act. Over the course of the last ten years, funding for the <br />Older Americans Act has increased by 16%. However, those needing meals has increased by 44% indicating an actual decrease in <br />funding. We are seeking financial support from the City of Santa Ana to assist us in closing the gap between funding and costs. <br />Because these programs target low income and minority residents, the donations received from the participants do not begin to cover <br />the costs. The average donation for the senior lunch program in Santa Ana is $1.53. Feedback is seeking $24,000 for the Senior <br />Lunch Program that will be used to directly cover food costs. <br />Exhibit A <br />Page 1 of 2 <br />
