Description of Outcome Tracking System: <br />Please describe your organization's outcome tracking system that will enable you to fulfill the outcome selected above. <br />Feedback Foundation's Senior Lunch Program's objective is to alleviate poor nutrition and isolation among the vulnerable 60+ <br />population in Santa Ana through adonation-based daily (Monday through Friday) lunch program at three(3)congregate dining centers. <br />The lunch program provides a focal point for seniors where they can come together in a supportive environment for awell-balanced <br />noon meal. The Lunch Program offers a comfortable link to other services and activities that keep seniors connected to resources and <br />the community. We anticipate serving 31,000 lunchtime meals to 880 seniors in Santa Ana during 2005/2006. <br />Methodology <br />Objective: <br />1. To alleviate poor nutrition among low-income elderly and assist these participants in remaining independent and in their own homes. <br />Task/Action: Provide lunch at three different locations in Santa Ana for senior to eat nutritiously and socialize. <br />2. Reduces social isolation through daily contact and monitors clients' health and functioning and links the client to other resources. <br />Task/Action: Site Managers, who are trained to be aware of clients' physical and mental well being, will be in attendance manageing <br />the programs daily. <br />3. A hot nutritious lunch is provided in a safe and friendly environrent, which offers active older adult opportunities for social <br />interaction and nutrition education. <br />Exhibit A-1 <br />Page 3 of 4 <br />
