City of Santa Ana <br />Community Development Block Grant <br />Scope of Work <br />Name of Organization- Orange County Bar Foundation (OCBF) <br />Name of Funded Program- Stop Short of Addiction (SSOA) <br />Annual Accomplishment Goal <br />I. Total number of unduplicated clients (Santa Ana and Non-Santa Ana Residents) anticipated to be served by <br />the funded program, named above, during the 12-month contract period. <br />120 (60 youth and 60 narents)_PERSONS <br />II. Number of only unduplicated Santa Ana residents to be served by the funded program, named above, <br />during the 12-month contract period. <br />24 residents (12 youth and 12 parents~PERSONS <br />Program and Funding Description <br />III. Description of Work - In the space below, describe the program to be funded during the 12-month <br />contract period. <br />Services to be provided: Stop Short of Addiction -Spanish program (SSOA-Sp) includes a clinical intake assessment, three pre- <br />therapy sessions that are psychoeducational and process oriented, up to 12 sessions of family therapy as resources allow, and an exit <br />meeting and plan to assist youth and their families in maintaining a drug and alcohol free life. <br />Program Goals: The goal of SSOA-Sp is to reduce risky behaviors such as substance abuse, while linking Latino youth and families <br />to culturally appropriate healthcare services. High-risk adolescents between 12-17 years and their families to accomplish the <br />following objectives: 1) provide access to free substance abuse program that is culturally, language and age appropriate ; 2) work <br />with underserved, Latino youth to reduce the spread of substance abuse, and 3) provide information critical to accessing substance <br />abuse and behavioral health services. <br />Clients served are Latino adolescents bewteen 12-17 years and their families. Our client's have limited English speaking abilities, are <br />oflow-income status, limited educations and lack health insurance. Our adolescents are at high risk of pregnancy, drug/alcohol use, <br />dropping out of school and exposure or involvement in gangs. <br />IV. Activities funded by this grant -How will CDBG funds be used for the funded program? Narratively <br />describe the budget for the funded program illustrated in Exhibit B and B-1. <br />Grant funds will go toward funding 15% of SSOA Coordinator salary. <br />SSOA Coordinator, Martha Veliz, is bilingual-bicultural and is repsonsible for providing clinical intakes for SSOA participants as <br />well as linking them to necessary resources and services. Additionally her responsibilities include providing Latino youth and <br />families with prevention treatment and follow-up services. She works with local police and schools on referrals and provides them <br />with follow-up reports. She is also in charge of program materials, scheduling facilities and instructors, and client demographic data <br />collection. Martha will spend 35% of her time on the SSOA Santa Ana referrals, however, we are asking that CDBG - SA cover 15% <br />of her salary. <br />Exhibit A <br />Page 1 of 2 <br />
