<br />month, mentors will be instructed to meet with the youth additional hours <br />during the first two weeks after intake since these are the most critical in <br />the transition process. <br /> <br />} Serving Foster and Emancipated Youth - Emancipated and Foster <br />youth are disproportionately represented among out-of-school youth. <br />These youth have higher rates of teen pregnancies, a higher prevalence <br />and incidence of substance abuse, and are generally at greater risk than <br />their colleagues who live with one or both parents. However, cognizant of <br />such social realities, services unique to the needs of this target group are <br />available. CHCADA will include specialized case management/referral <br />services for these youth. <br /> <br />} Involving the Parents of Participants - FUERZAS UNIDAS' counselors will <br />meet with the partents of participants on a monthly basis to review <br />progress and identify ongoing problems with the youth and offer input <br />relative to the management of these problems; and to discuss the overall <br />progress of the youth. As required, in-home meetings will occur with the <br />parents and family of each youth at minimum once a month, but as often <br />as necessary for the needs of the youth. <br /> <br />Timely exit to meet WIA performance Finally, staff will maintain attendance <br />books/sign-in sheets on all of the above activities. At the conclusion of the <br />program, a narrative report will be input into the vas system, providing all <br />required data. In addition, Counselors will document in the client file the youth <br />and his/her family's participation in all required and non-required programmatic <br />components - assessing the youth's progress relative to the Case Plan and <br />making recommendations regarding additional interventions. At the end of the <br />program, a formal graduation ceremony will be scheduled and Certificates of <br />Completion/Attendance will be awarded. <br /> <br />Task 7: Collection, Maintenance, Storage, Retrieval, and Reporting of Applicable <br />Data to WIA <br />The Program Coordinator will assume the responsibility of serving as Data <br /> <br />Management Coordinator for the WIA participants. In this capacity, the Program <br /> <br />Coordinator will: <br /> <br />o Ensure that CHCADA keeps copies of all files for all youth admitted into the <br />program; <br /> <br />o Complete all CHCADA Intake Forms for participants and enter these into their <br />files; <br /> <br />30 <br />
