Description of Outcome Tracking System: <br />Please describe your organization's outcome tracking system that will enable you to fulfill the outcome selected above. <br />The Cold/Wet Weather Emergency Shelter (Armory) Program provides homeless and chronically homeless adutlts and families <br />with emergency shelter services, access to essential services and, when appropriate, individualized case management. Service delivery <br />data is recorded for HUD Specific Outcomes Indicator numbers 12 and 13. Data is recorded on a nightly basis on Emergency Cold <br />Weather Shelter Program Sign -In forms and a Daily Activity Report form. Sign -in forms record the client's name, date of birth, gender, <br />status as a new or returning uses of shelter services for the current year, marital status, number in family, status as head of household, <br />veteran status, employment status and ethnicity. Daily Activity Report forms record staff names and schedules, number of volunteers, <br />new and repeat client census by gender and adult/juvenile status, meals delivered, program operational needs, and incidents worthy of <br />documenting for subsequent review if needed.. <br />Exhibit A-1 <br />Page 3 of 4 <br />
