Part Three — Application Requirements and Process <br />Application The application must include the following: <br />Components <br />• Application for Assistance <br />• Project Narrative <br />• Project Budget Summary Worksheet <br />• Governing Body Resolution <br />• Grant Assurances <br />Mailing The application must be mailed to your Regional office: <br />Address <br />Coastal Region Inland Region Southern Region <br />1300 Clay Street, Suite 408 3650 Schriever Avenue 4671 Liberty Avenue <br />Oakland, CA 94612 Mather, CA 95655 Los Alamitos, CA 90720 <br />Application for The Application for Assistance form is the coversheet for the application. It <br />Assistance identifies the applicant, the authorized agent and contact person for the grant <br />and their addresses, the maximum award amount and the amount the OA is <br />requesting. It must be signed by the authorized agent and, after review, it is <br />approved by OES. This document serves as the contractual basis for the <br />award of funds. <br />Project The project narrative must include the following: <br />Narrative <br />• An OA assigned number for the project. <br />• A project title and description. <br />• The goal and objective (from Appendix B) that the project supports. <br />• Projects should be presented in terms of what will be accomplished. <br />For example, if training is planned, the jurisdiction must indicate what <br />the training is expected to accomplish. The jurisdiction should clearly <br />indicate how it would know its purpose has been accomplished. <br />The project narrative forms the basis for the performance reports and OAs <br />will be expected to report progress on each project. <br />Continued on next page <br />FY 06 EMPG Grant Guide for Local Govemment Page 20 of 51 11 <br />
