ARTICLE XI1 <br />12.0 EMPLOYEF, INSURANCE <br />12.1 Health Insurance. The City shall contribute the following amounts toward the payment of <br />premiums for affected employees and their dependents under the California Public <br />Employees' Retirement System (CaIPERS) health insurance programs. <br />A. Effective March 1, 2005, January 1, 2006, January 1, 2007, January 1, 2008, January <br />1, 2009, and January 1, 2010, respectively, the City shall contribute toward medical <br />premiums an amount consistent with the rates then in effect for the "employee-only" <br />and "family" tiers, respectively, of the CaIPERS Kaiser "Other Southern California" <br />HMO plan. The "employee only" tier applies to employees who have no <br />dependents. Effective January 1, 2006, the City established a Cafeteria Benefit Plan <br />for employees covered by this Agreement. <br />B. Any contribution necessary to maintain benefits under any health insurance program <br />provided by the City for its employees and their eligible dependents in excess of the <br />amounts of the City contribution specified above shall be borne entirely by the <br />employee. <br />C. For each such employee who is covered under a spouse's non-City sponsored health <br />plan, the City will pay the employee a cash payment each month in an amount equal <br />to one hundred percent (100%) of the monthly premium amount for the City's lowest <br />"employee-only" coverage, if said employee waives, in writing, City-paid coverage. <br />If an employee waives City provided coverage, said employee shall provide proof of <br />medical insurance coverage in a non City-sponsored health plan. Said waiver shall <br />include a provision warning such employee that reentry into any of City-sponsored <br />plans is allowed only at open enrollment, unless there is a qualifying event, and may <br />require proof of insurability for such employee and/or family. <br />12.2 Dental Insurance. The City agrees to contribute toward the payment of premiums for dental <br />insurance plans provided by the City for employees covered by this Agreement and their <br />eligible dependents on the following basis: <br />Effective March 1, 2005: <br />A. One hundred percent (100%) of the premium cost for "employee-only" coverage. <br />B. tJp to eighty dollars ($80) per month per employee for "family" coverage. <br />34 <br />
