<br />Part Four - Performance Period Activities, Continued <br /> <br />Performance <br />Reports <br /> <br />Grant recipients must prepare and submit performance reports to the state for <br />the duration of the grant period, or until all grant activities are completed and <br />the grant is formally closed, whichever comes first. The reports must include <br />the status of all projects, as well as other requested information and data, and <br />are due as follows: <br /> <br />. Reporting Period January I through June 30 - due by July 15 <br />. Reporting Period July I through December 31 - due by January 15 <br /> <br />A performance report template, with instructions, will be distributed under <br />separate cover prior to the deadline for the report. Failure to submit <br />performance reports could result in grant reduction, termination or <br />suspension. <br /> <br />Interest <br />Earnings <br /> <br />In accordance with Title 28, Code of Federal Regulations, Part 66.21, and <br />DOJ Financial Guide, Part III, Chapter I, any interest earned, in excess of <br />$100 per federal fiscal year, must be remitted quarterly to: <br /> <br />United States Department of Health and Human Services <br />Division of Payment Management Services <br />P.O.
