Services are performed <br />at the designated Motorola facility. <br />In addition to Equipment specifically named in the applicable Agremem to which this Statement of Work is attached, <br />Radio Repair uwtudes service ea standard palm microphones and single mobile control heads provided that they are <br />requiad fur normal operation of the Equipment and em included at the point of manufacture. Additional mobile control <br />heads ate covered only with purchase of the applicable service option. <br />Radio Repair excludes repairs to; optional accessories; non-standard mobile microphones; iDEN accessories; iDEN mobllc <br />microphones; portable remote speaker microphones; optional or additional control heads; mobile external speakers; single <br />and multiple unit portable chargers; batteries; mobile power and antenna cables; mobile ante®as; portable antemas, and <br />power supplies. New Equiptrtetrt purchases will only be added upon (lrstomer request. <br />The tcmu and conditions of this Statement of Work are as integral part of the Motorola Service Agreement or other <br />applicable Agratncnt to which it is attached and made a part thereof by this reference. [f there are any inconsistencies <br />between the provisions of the MotoroL•r Service Agreement or other applicable Agreement and this SOW, the provisions of <br />this Statcmcat of Work shaE prevail. <br />.- 2.0 Motorola has the following responsibilities: <br />2.l Test and Restore the Equipmeat to Motorola factory specificahoas. <br />2.2 Reprogram Equipment to origino] operating parameters based on the Customer terrgtlate, if retrievable, or fiom a <br />Customer supplied backup diskette. If the Customer template is not usable, a generic template utilizing the latest <br />Radio Service Software (RSS) version for that Equiptttertt will be used. The Equipment e-ill require additional <br />programming by the Cusromer to restore the original template. <br />2.3 Clean external hotuing ofthe Equipment. <br />2.4 Pey the outbound fieigbt charges for next day shipping. <br />2.5 Pay the inbound Height charges oNy if the Customer uses the Motorola designated delivery service. <br />2.6 Provide Qutoaxr will[ the Motorola repair request forth and inventory adjusmreat form <br />2.7 Process inventory adjustrrrent requests received by fax or email from Customer. If the request is reteived by email, <br />Motorola will entail an acknowledgement to the sender. <br />2.8 If applicable, testify Customer of changes in Motorola designated inventory adjustrnent email address or fax number. <br />3.0 Customer has the following Respotrsibilities: <br />3.1 Utilize the Motorola designated delivery service program for inbound shipphtg. <br />3.2 Pay freight iasuranee charges fur declared values is excess of ~ 100 on inbound shipments. <br />3.3 Complete a Mutomla repair request form and submit it with each unit of Egoiptnent sent in for service. Mobile <br />control heads or acttssory items sent is must reference the serial number of the Hain unit. <br />3,4 If desired, supply Mommla with a 3.5"backup diskette with the Software template or prvyerarmrting in order to assist <br />in returning the Equipment tv original operating parameters. This step mutt be completed for Equipment that will not <br />power up.
