City of Santa Ana <br />Scope of Work <br />Name of Organization VIETNAMESE COMMUNITY OF ORANGE CPUNTY, INC. <br />Name of Funded Program SEA Youth Gang/Crime Prevention <br />Annual Accomplishment Goal <br />I. Total number of unduplicated clients (Santa Ana and Non -Santa Ana Residents) anticipated to be served by the <br />funded program, named above, during the 12 -month contract period. <br />52 Persons <br />II. Number of ONLY unduplicated Santa Ana residents to be served by the funded program, named above, during the <br />12 -month contract period. <br />21 Persons <br />Program and Funding Description <br />III. Description of Work - In the space below, describe the program to be funded during the 12 -month contract period. <br />What specific activities will be undertaken during the contract period. Please be concise in your response. Only the <br />VICWdUIe SpdUt: WI11 [AIfR. <br />During the 12 months period, I, Thinh Tran will recruit clients from many locations such as Los Pinos Convervation <br />Camp, Joplin Youth Center, and posibly Valley High School,Youth Detention Center in Santa Ana and throughout <br />Orange County. I will also receiving Youth -at -Risk through Deputy Probation Officers, students of high risk, low school <br />grade living in Santa Ana. <br />The client recruited also would be from Santa Ana schools. Such youth identify by parenst as youth as risks. All of <br />them whose parents are low or very low income. We monitor their progress with Intake form, communicate with their <br />parents, asking them to provide grade report. <br />We provide counseling in areas of Anger management, Identifying and Handling Peer Pressure , Self Esteem <br />Building, Realizing Importance of Education., Drugs prevention, Rights and Responsibility of Youth, Road to Self - <br />Sufficiency, and Goal setting. Counseling in the form of verbal communication and hand-outs. <br />Obviously, some youth are brighter than other, thus different level of understanding and need arises with each youth. <br />Some need more emphasis on certain topics than others. <br />We are confident we can meet or exceed our original goal of 52 youth. Once we can gain acess more into juvenile <br />centers through Orange County to locate Santa Ana youth, the current number will certainly be increase. <br />ocneuule or rerrormace <br />Estimate the number of ONLY unduplicated Santa Ana residents to be served by the funded program during the 12 - <br />month contract period per quarter. (Enter number of new Santa Ana clients served each quarter.
