In <br />addition, income, race, and single mother information is gathered, as well referring staff from the school. The <br />accompanying guardian's signature is obtained on the day of the visit, after verifying the information on the form. This <br />information is backed up with a dated schedule which reflects the show/no show rates, and any substitutions made. <br />OSB@ maintains an alpha organized (by mother's last name) file boxes with cards for each family served. Each card <br />reflects the child served, date, and the name of the workers that served that child.
