Center <br /> <br />Program and Funding Description <br />III. Description of Work - In the space below, describe the program to be funded during the 12-month contract period. <br />What specific activities will be undertaken during the contract period. Only the <br />viewable space will print. <br />The Health Consumer Action Center (HCAC), a program of the Legal Aid Society of Orange County (LASOC), <br />provides legal advocacy and expertise to assist low income and underrepresented consumers access the Orange <br />County health care system. The purpose of HCAC is to assist low-income and underrepresented consumers access <br />quality health services. As the only program of its kind in Orange County, HCAC provides legal advocacy and <br />expertise to equip healthcare consumers with vital information, enabling them to obtain coverage and benefits. <br /> <br />The objective of the program for which we are requesting Santa Ana CDBG funding is to assist 98 low income <br />residents of Santa Ana resolve problems related to accessing critical medical services. This objective will be met by <br />HCAC acting as legal advocate for these Santa Ana residents to ensure they receive much needed medical care <br />regardless of income. The actions involved in this objective on a day-to-day basis are: screening applicants for <br />eligibility through the HCAC Hotline; providing immediate advice and counsel to eligible clients; and legal <br />representation for those clients who are in need of an attorney to intervene on their behalf. The second objective of <br />HCAC is to educate low-income Santa Ana residents on their rights under current laws and regulations, particularly as <br />it relates to managed care. This is accomplished by providing information to eligible clients regarding their specific <br />healthcare provider and insurance program. The third objective of HCAC is to enable low-income Santa Ana residents <br />to become more active participants in their own healthcare. This is done by explaining to eligible clients what options <br />are open to them, what benefits they are entitled to receive, and what specific actions they can take to solve their <br />problems. <br /> <br />Schedule of Performace <br /> <br />Estimate the number of ONLY unduplicated Santa Ana residents to be served by the funded program during the 12- <br />month contract period per quarter.
