Description of Work - In the space below, describe the program to be funded during the 12-month contract period. <br />What specific activities will be undertaken during the contract period. Please be concise in your response. Only the <br />viewable space will print. <br /> <br />(1) Emergency Dental Assistance. The Foundation will continue to fund emergency dental care to Santa Ana <br />residents through reduced cost services performed by Jay Bauman DDS, a Santa Ana dentist. The Foundation also <br />works with the Lenscrafters "Gift of Sight" program to obtain free eye exams and free eye glasses for participants <br />including Santa Ana residents. <br />(2) Transportation Vouchers. The Foundation will continue to provide Santa Ana residents with OCT A bus passes to <br />enable them to attend mandatory appointments as well as school/work. <br />(3) Opportunities to satisfy cultural/civic activities requirements. The Foundation will continue to offer Santa Ana <br />residents the opportunity, at no cost to the participant other than his/her transportation expenses, to satisfy the <br />requirement to engage in cultural, civic and educational activities. These activities include specially designed tours of <br />the Getty Museum, the Orange County Art Museum, the Laguna Canyon Wilderness Park and the Upper Newport <br />Bay. <br />(4) Link participants with existing resources. The Foundation will continue to help link Santa Ana residents/participants <br />with existing publicly and privately funded agencies such as the LenCrafters "Gift of Sight Program" that provide eye <br />exams & glasses at no charge, Women Helping Women that provide clothing and assistance finding employment <br />,Working Wardrobes that assist with clothing and building self-esteem. <br />(5) Form and support alumni programs. The Foundation helps graduates form alumni groups and works with those <br />groups to build membership, become mentors to participants and help educate others. <br /> <br />Schedule of Performace <br />Estimate the number of ONLY unduplicated Santa Ana residents to be served by the funded program during the 12- <br />month contract period per quarter.
