(WfLC) <br />Independence from Dependence <br /> <br />Program and Funding Description <br />III. Description of Work - In the space below, describe the program to be funded during the 12-month contract period. <br />What specific activities will be undertaken during the contract period. Please be concise in your response. Only the <br />viewable space will print. <br /> <br />The "Independence from Dependence" program is a supportive services and emergency shelter program for victims <br />of domestic violence who may also be chronically homeless and/or victims of human trafficking - with or without <br />children, substance abuse and/or mental health problems. The goal is to identify and assist victims of domestic <br />violence and/or human trafficking in becoming free, safe, independent members of society. <br /> <br />Outcomes include: a) increasing levels of safety and stability through provision of shelter, supportive services, and <br />increased access to resources; b) increase independence through provision of services and collaboration with other <br />agencies; and c) reunification with other family members (in cases of human trafficking). <br /> <br />Specific activities include: 1) substance abuse - a) education for 200 adults and 150 children, b) treatment for 20 <br />adults and their children; 2) violence issues - a) education for 2,000 adults and children, b) counseling for 100 adults <br />and children; and 3) establishing permanence - a) resource assistance for 250 adults and children, b) safety planning <br />for 100 adults, c) establishing independence plans for 100 adults, and d) helping undocumented clients obtain <br />documentation and become reunited with their families. <br /> <br />Evaluation tools include: pre/post-tests for educational classes, the client database ("Alice") which outlines all client <br />information, demographics and services provided, client files and drug test results. <br /> <br />Schedule of Performace <br />Estimate the number of ONLY unduplicated Santa Ana residents to be served by the funded program during the 12- <br />month contract period per quarter. (Enter number of new Santa Ana clients served each quarter.
