The discussion about housing includes two major considerations: what the resident is
able to afford and if they are able to live on their own in an apartment, etc. For those who are ready to live on their own,
the FEM assists them in finding a safe, affordable place in an apartment, a room in a large home, etc. If they aren't
ready to live on their own, they are referred to other, longer -term transitional housing programs. In addition to weekly
meetings with the FEM, every resident also meets with her Case Manager on a weekly basis. Notes of these meetings
are kept in each resident's files as well as entered into the CMIS (Client Management Information System.) When a
resident leaves our program, her forwarding address, location of her job and her monthly income are entered into her
file, both paper as well as CMIS. All program graduates are invited to participate in our monthly Alumni Support Group.
Finally, each program graduate will receive a survey, after she has been out of the program for one year, to track any
changes in her job, monthly income, home address, etc.
Exhibit A -1
