Describe how you will gather the tracking information, noted above, for that activity. <br />One of the primary goals for all of our residents is to find a safe, affordable place to live when they leave our program. <br />As they come to the end of their stay in our program, housing is addressed in their weekly meetings with the Financial <br />Empowerment Manager (FEM.) The discussion about housing includes two major considerations: what the resident is <br />able to afford and if they are able to live on their own in an apartment, etc. For those who are ready to live on their own, <br />the FEM assists them in finding a safe, affordable place in an apartment, a room in a large home, etc. If they aren't <br />ready to live on their own, they are referred to other, longer-term transitional housing programs. In addition to weekly <br />meetings with the FEM, every resident also meets with her Case Manager on a weekly basis. Notes of these meetings <br />are kept in each resident's files as well as entered into the CMTS (Client Management Information System.) When a <br />resident leaves our program, her forwarding address, location of her job and her monthly income are entered into her <br />file, both paper as well as CMTS.
