A hotline call form is <br />completed by the hotline staff for each call. The call form is designed to gather the following information: date & time of <br />call; caller's immediate situation - need for shelter or other supportive services; demographic data such as name,age, <br />gender, race, ethnicity, household size, income level, dispostion of call and referral resources the caller was linked to for <br />services sought. <br /> <br />This data is entered into a monthly spreadsheet to create a summary of the demographic data on each call received. <br />This information is then readily available for providing statistical data to program directors and funders for review and <br />verification of services provided. <br /> <br />Exhibit A-1 <br />Page 3 of 3 <br />
